i may be being fantastically dense here, but are you suggesting that she was... pregnant? if so, i guess that can account for why she took off and didn't stick around to make aquaitance with wendy's oversized pet. I seriously doubt it would come to that, more likely it would be coathangers down a back alley time. (And if you don't get that... you probally don't want to know) Edit, as I didn't see Bucketknights post: I take this as to have happened long before Hoffman left, that Clara realised she was pregnant and he then performed an abortion on her. The whole event would have been hushed up and talking about it around any of the adults would have been a very bad idea, still I bet just about everyone knew about it. I doubt the event did much for the poor girls already shaky mental state, it might explain why she left.

My opinion is that the "vomit" is a simple metaphor for "disgust".
Clara was disgusted by Hoffman, by the whole orphanage (the children probably were not friends of hers, even if they didn't mistreat her - nobody seems to worry about Clara, aside Hoffman) and by herself too. Sometimes abused children think wrongly that they are partially responsible for *what* happened; inside their minds they see themselves as guilty. One day, her disgust for the whole thing (for Hoffman, for the children that didn't help her, for herself...) was too much, so she left the orphanage forever, even if Hoffman was already gone. Pregnant? I don't think she was, but it's an interesting theory.
Why did Clara attack Jennifer in mermaid shape? No particular reason, I suppose. In my opinion, Clara'd have attacked every person trying to *go* inside her bed! She 'vomits' all around the hate and the disgust accumulated inside her.
I have a doubt. Could Clara and Diana be related? They share the same chapter and their faces look very similar. Perhaps Clara was Diana's older sister?
