1 - Silent Hill 3 and 4. Can't play these myself, only watched someone play.
2 - Resident Evil series. Zombies really freak me out. The Regenerator from RE4 is the scariest thing ever. Zoom in on its face with the rifle.
That's all really. There are others but they're more the types that aren't scary anymore.

1- Silent Hill 3: I don't think a game as frightening and gory as this one will ever be made again...
2- Fatal Frame 3
3- Silent Hill: it's just... so scary. specially the school chapter
4- Clock Tower 3: Crazy stalkers...
5- Rule of the Rose: when you don't know what's going on this is a really scary game

I would have put CT3 in my list, but it only scared me the first time. And even then only the first 2 stalkers. The rest started to get weird, like circus freaks or something.
Guess I'll add Siren to my list. I forgot because I hadn't played it for so long. Kuon freaked me out too, but once again only the first time.

yea i like die on the floor whenever im being chased, the settings are like complete mazes, i especially hate it that when youre in the middle of something important, and out of nowhere your running around the whole stinkin place then you have to back track all the way back to that place that you actually needed to be at. and that darn map is completely useless.

1.Silent Hill-This was the first game I'd every played that caused me to have a nightmare.
2.Fatal Frame III- Try walking down the stairs to hear the shrine maidens counting, and those girls just generally make the game horribly creepy.
3.Clock Tower 3- God I don't think anythings ever made my heart race the way the hammerman did.
4.Eternal Darkness- It gave my Gamecube an Operating system error, Omg.
5.Rule of rose- I keep hearing the imps chanting. Even after I've stopped playing it.

Scariest games?
i'll say
-Resident Evil outbreak file 2 .... i know "what the heck why RE outbreak?" i have a reason for this and i really don't wanna say it cuz it's embarrassing
-I forgot the the name, but it's a japanese game and the only reason i got scared of that game is cuz i don't know how to save....
