If Hoffman, Martha, and Clara had not left, do you think the massacre still would have happened?

It depends on if they would have been able to stop Wendy from leaving the house I think. Someone might have possibly called the cops too and kept the children inside.
But I doubt it, Hoffman was a horrible caretaker.

I don't think they could have stopped Wendy from leaving, since they've got a lot of kids too look after, and the orphanage is big. Also, we know she has managed to sneak out many times before in order to visit Jennifer. The unused dialogue hints that after Wendy lost her rank, she disappeared, which possibly means up until she reappears with Stray Dog. If the adults were there, the might have been able to contact the authorities and prevented Wendy from bringing Gregory to the orphanage. They would not have had any trouble finding her, due to Martha spotting her with Gregory earlier and him being the only person living in that area. Of course, it would seem Gregory was already Stray Dog at this point regardless. If the adults were around, Wendy could switch stragedies and keep going between the orphanage and Gregory's house.
As for the massacre itself, it happened at night, when everyone should have been in bed. Unless Wendy switched it to daytime for some reason, she would easily be able to sneak Gregory in and would know where all the kids are to sic him on them. Even if he could not take down the adults, at least some of the kids would have died. We know they would be too scared to run, but maybe those on the top bunks would be out of reach.

Gregory seems tall enough to reach the top bunks easily enough, but those kids would have a head start. At least maybe some children would have survived, or an adult could have seen Wendy with Stray Dog approaching and thought "oh no you din'nt". I still don't understand why they just ran off. That's pretty shameful. Hows that for a guilty conscience afterwards?

I don't think the kids would have ran, as they seem to have froze with shock and terror outside when it happened. While I have no doubt Gregory would have been tall enough, he was acting like a dog, so I can't just see him standing up all of a sudden.

Wendy is capable of anything, she might just command Gregory to kill the adults first then the children. She already had plans to eliminate the entire orphanage. Then again, it would be interesting to see how each of the adults would react if Wendy brought Stray Dog to the orphanage that night.

Well, I feel certain Hoffman would abandon the children and flee to save his own life, which he kinda does in the game. Martha might do the same, though she does express fear for the kids' safety, she is also the one who leaves them completely adultless-at least Hoffman left knowing she and Clara were there. I picture her being cornered and killed by Wendy and Stray Dog, same as Clara.

I'm not sure, I can see Clara herding some of the children out while Stray Dog attacks Martha or Hoffman. It really depends on where Wendy goes first, but at least one person is certain to die.

I dunno, some people freeze in emergencies and some get proactive. There's no real telling who will do which until it happens.
